CRC will provide forms and walk you through both the application and direct billing process to fund your equipment and supplies. We will use our extensive knowledge of both private, government and insurance carriers to assist in your application for any additional coverage that may be available to you.
CRC also offers solutions for those who do not qualify for government funding, and do not have Health Insurance plan coverage.

AADL Client Declaration Form (To be completed before an order can be placed)
AADL Client Responsibility Form (Clients wanting benefits must meet the current AADL respiratory eligibility criteria)
Insurance Funding
Private funding may also be available and Chinook Respiratory Care offers alternative payment plans, Rental, and Rent-to-Own arrangements if other funding is not available to you.
